Important note: If the seller is unable or unwilling to provide you with the basic information requested by this form, this is a RED FLAG. I highly recommend buying from a small hobby breeder who is passionate about the care of their animals. These people often have fewer animals and limit themselves to one or just a couple species. Responsible re-sellers are also an option and can be identified if they can answer specific questions about the animal and its actual breeder. Re-selling does come with an inherent risk of virus/disease transmission, so it is important to ask if the re-seller (and their sources) health test and practice good quarantine procedures. Be aware of any vendor with numerous tables filled with a large array of species. These are flippers/distributors who buy animals wholesale and have no concern about communicable diseases or quarantining different species. They will also have little to no information on your purchased animal and may even give you incorrect information.
